The theory and practice of fat grafting in plastic surgery

INTRODUCTION
Fat grafting, also known as autologous fat transfer, is a procedure in plastic surgery where fat is harvested from one part of the body and transplanted to another area to enhance volume, shape, silhouette, or contour. It can also fill in dents or wrinkles or folds and be use to rejuvenate and youthify the skin. This technique has gained significant popularity due to its dual benefits: the removal of unwanted fat (by liposuction) and the natural enhancement or correction or rejuvenation of other areas. Fat grafting is used in various aesthetic and reconstructive procedures, offering a natural alternative to synthetic fillers and implants.

THE THEORY BEHIND FAT GRAFTING
Fat grafting is grounded in the principle of using your body’s own natural fat cells to rejuvenate skin, correct defects, or enhance areas with volume loss. Similar to the science behind skin grafting, fat is gently harvested by liposuction using very small cannulas and a low vacuum pressure to prevent harm to the cells. The cells are then prepared by washing and adding antibiotics and your own platelet rich plasma (PRP). This PRP “supercharges” the fat graft so that it heals in better and more survives. The fat is then transplanted into the recipient area and the new blood vessels grow into the fat cells. Once this occurs the fat cells and fat graft survive forever. The technique involves three main steps: harvesting, processing, and reinjection.

  1. Harvesting: Fat is always harvested through liposuction, where a small cannula is inserted into areas with excess fat deposits, such as the abdomen, thighs, or flanks. The fat is then suctioned using a syringe or a suction machine. Low pressure is used to decrease the injury to cells. Small cannulas are used so that the fat globules are small and facilitate better transplantation and blood vessel ingrowth. The big bonus here is that you get liposuction and removal of unwanted fat as a byproduct.
  2. Processing: Once harvested the fat is collected sterilely in a container, then next any fluid and blood is allowed to separate from the fat cells then antibiotics and PRP are added to the fat cell mixture. Finally, the fat in loaded into small syringes connected to very small diameter cannula. The goal is to harvest and then isolate healthy fat cells that can survive and integrate into the recipient site and enhance blood vessel ingrowth.
  3. Reinjection: The processed fat is then injected into the areas to be treated using very small ¼” incisions and placed in very small crisscrossing tunnels to slowly fill in the area very carefully and EVENLY and artistically. The technique used for reinjection is crucial for the survival of the grafted fat. Dr Romano uses state-of-the-art micro-droplet technique, where small amounts of fat are injected in different planes to ensure a good blood supply and minimize the risk of fat necrosis or resorption.

    • Breast Augmentation: Fat grafting offers a natural alternative to implants for breast augmentation. The amount of enhancement of the breasts depends on the amount of fat that is available as a resource. Fat grafting of the breast is also used in combination with implants to correct contour irregularities or asymmetries.
    • Body Contouring: One of the most popular procedures nowadays is where fat grafting is used to enhance areas such as the buttocks (Brazilian Butt Lift) or to correct contour deformities after liposuction.
  2. Reconstructive Surgery:
    • Breast Reconstruction: Fat grafting is often used after mastectomy to improve the contour and symmetry of reconstructed breasts or to correct defects following lumpectomy.
    • Scar Revision: Fat grafting can improve the appearance of scars or dents or depressions by adding volume and improving the quality of the overlying skin.
    • Hand Rejuvenation: Fat grafting is used to restore volume and improve the appearance of aging hands, especially on top to camouflage the visible tendons and large veins.

CONSIDERATIONS IN FAT GRAFTING
While fat grafting offers many benefits, it also has some limitations and challenges:

  1. Variable Graft Survival: One of the main challenges of fat grafting is the unpredictable survival of the transplanted fat. There is no prediction on the amount of successful transplantation and survival of fat cells, but typically, only 50-70% of the grafted fat survives long-term, which may require additional sessions to achieve the desired result. BUT, once the fat graft survives and the new blood supply grows in it will last forever. You can always build up more layers of fat enhancement on subsequent surgeries.
  2. Fat Resorption: As the revascularization process is occurring in newly grafted fat (the blood supply growing in), some of the grafted fat may be resorbed by the body, leading to a partial result of the initial grafted volume of fat. This can vary depending on the technique, the area being treated, and the individual patient.
  3. Complications: Fat grafting is generally safe, but it carries some risks, including infection, fat necrosis (where the fat cells die and form lumps), and the formation of cysts or calcifications. All of these can be addressed and cured by an additional small and predictable surgery if needed. There is also a risk of embolism if fat enters the bloodstream, though this is rare.
  4. Judgement and Communication: Successful fat grafting requires great judgement and communication. It is important for the surgeon to be able to expertly understand your expectations and then combine this with what your anatomy will allow safely and predictably to provide a result. Assessment, education, managing expectations and communication is essential for a good result in fat grafting. Of course, this is also true for all of plastic surgery.
  5. Technical Skill: Successful fat grafting also requires great technical skill and expertise. The surgeon must carefully harvest, process, and reinject the fat to maximize survival and achieve a natural-looking result. Inexperienced practitioners may have higher rates of complications or unsatisfactory outcomes.

ADVANCES IN FAT GRAFTING TECHNIQUES
Recent advances in fat grafting techniques have aimed to improve graft survival and reduce complications:
